As a Licensed Practical Nurse, you will be a crucial clinical team member providing direct, compassionate care and supporting the health and well-being of our residents.
- Medication Administration: Accurately, safely, and timely administer all prescribed medications to residents according to their individual care plans and physician orders.
- Treatment Execution: Perform and monitor complex treatments as directed by physicians' orders and strictly within your defined scope of practice.
- Clinical Observation & Reporting: Meticulously observe, record, and report any changes in a resident’s physical, cognitive, or emotional condition, including new symptoms or vital sign fluctuations, to the Registered Nurse or Supervising Physician promptly.
- Care Plan Support: Assist the Registered Nurse with implementing and documenting the resident care plan, ensuring CNAs are performing tasks correctly.
- Documentation & Compliance: Maintain accurate, detailed, and timely electronic health records (EHR) and ensure all duties are performed in strict adherence to local, state, and federal guidelines (including HIPAA) and facility policies.
- Team Collaboration: Work effectively as part of the interdisciplinary team, while also being capable of exercising independent clinical judgment when necessary.
- License: Must possess an Active Licensed Practical Nurse (LPN) license in the state of Wisconsin in good standing.
- Skills: Strong communication and organizational skills; demonstrated ability to provide compassionate, high-quality care.
- Technology: Familiarity with Electronic Health Records (EHR) and basic computer systems is required.
- Experience: Prior experience in a long-term care or skilled nursing setting is preferred but not required. We are committed to your success and will provide the training you need.
